Emplify Health by Bellin: Direct-to-employer model delivers $9.1 million in savings

GREEN BAY — Emplify Health by Bellin’s direct-to-employer care model delivered $9.1 million in savings to just 14 of its 112 employer partners in 2025, bending the trend of rising healthcare costs while working to improve health and satisfaction for the 27,000 employees who were part of this proactive, integrated approach to care.

The direct-to-employer leverages direct primary caredirect physical therapy and comprehensive population health services to close gaps in care – such as cancer screenings – and deliver exceptional outcomes in managing chronic conditions like blood pressure and diabetes.

Regional average healthcare costs were expected to rise 10.05 percent during calendar year 2025, according to the M3 Employee Benefit Trend Report. Emplify Health by Bellin’s goal was to reduce that to 8 percent, and ultimately their partners achieved a 5.25 percent trend, or 4.8 percent less than the average increase.
