Lakeland Care District: Family Care opens in Northeastern Wisconsin

Program available beginning June 1 in Kewaunee, Oconto Counties

(Green Bay) Beginning today, Family Care services and supports are now available to residents in Kewaunee and Oconto counties in Northeastern Wisconsin. Family Care, Wisconsin’s successful reform program providing managed long-term care supports and services to qualified elderly and people with disabilities, is now being implemented throughout Northeastern Wisconsin.

“Northeastern Wisconsin families have waited many years for Family Care,” said Katie Mnuk, CEO for the Lakeland Care District. “Once fully implemented, Family Care ends local waiting lists for needed assistance,” she said. “We’re excited to bring the Lakeland Care District’s tradition of individualized, cost-effective and high quality supports and services to people in these additional counties.”

Family Care is a Medicaid-funded program providing cost-effective, comprehensive supports and services in an individualized manner based on people’s needs. “Family Care supports people’s independence by providing the right services where and when people need them,” Mnuk explained. “We support people to remain as independent as possible while living in their homes and communities.”

The Lakeland Care District, a regional Managed Care Organization based in the Fox Valley, today begins providing Family Care to people in Kewaunee and Oconto Counties, two of seven counties opening Family Care this year. Later in 2015, the program will open in five other Northeastern Wisconsin counties including Brown, Door, Marinette, Menominee and Shawano. Eligible residents in these five counties will continue to be served by their county waiver systems until Family Care opens there.

The Lakeland Care District is opening offices in Crivitz, Algoma, Ashwaubenon and Shawano to serve Northeastern Wisconsin residents. The District already provides Family Care to eligible people living in Outagamie, Calumet, Manitowoc, Waupaca, Winnebago and Fond du Lac Counties. The District grew from Wisconsin’s first Family Care pilot started in 2000 in Fond du Lac County, and has successfully supported thousands of members over its last 15 years.

With today’s opening, Family Care is now available in 59 Wisconsin Counties. For more information on long-term care supports and services including Family Care, people should contact their local Aging and Disability Resource Centers.
