SHEBOYGAN, Wis. — Prevea Health invites the community to an Open House at its newest health center in Sheboygan located at 1703 N. Taylor Dr., on June 16, from 4:30 to 6:30 p.m.
Members of the community are encouraged to stop in for self-guided tours and to learn more from physicians and providers about the services offered there, including Family Medicine, Internal Medicine, Urgent Care, Occupational Health, Behavioral Care, Endocrinology, Lab and
X-ray. Light appetizers and refreshments will be served.
This new building represents a significant investment in the Sheboygan community, consolidating services currently offered at the Prevea Sheboygan Health Center at 1411 N. Taylor Dr. While the 1411 N. Taylor Dr. location will close its doors June 17, patients will continue to receive the same high-quality care at the new 1703 N. Taylor Dr. location starting June 22.
“Relocating to this building on the campus of our hospital partner, HSHS St. Nicholas Hospital, furthers our commitment to providing the Sheboygan community with easy access to essential health care services,” said Dr. Ashok Rai, President and CEO of Prevea Health. “This means a more connected healthcare experience for our patients, having a health care center equipped to serve a wide variety of needs.”
This new health center is now the second Prevea Health facility to join the HSHS St. Nicholas Hospital campus. The Prevea Sheboygan Medical Office Building at 3113 Saemann Ave, which offers a wide variety of routine and specialty care services, has also been a part of the hospital campus since 2019.
Since April 2026, all Prevea therapy services in Sheboygan – including physical, occupational, and speech therapy services– have been consolidated to the Prevea Sheboygan Medical Office Building. Starting June 8, OB/GYN services in Sheboygan will also transition to that location.
