Madison, Wis. — The Office of the Commissioner of Insurance (OCI) is responsible for administering and enforcing the insurance laws of Wisconsin. This includes monitoring the financial and marketing practices of individuals and companies. Each month, OCI publishes the administrative actions it has taken against these entities. The actions are issued by OCI when licensing applicants, or licensed individuals or companies have violated Wisconsin insurance statutes or regulations. In some cases, the individual or company denied the stated allegations but consented to the action taken. OCI strives to ensure fair and honest business practices to protect Wisconsin insurance consumers through this type of transparency. Any forfeitures paid are deposited in Wisconsin’s Common School Fund administered by the Board of Commissioners of Public Lands. Earnings from the Fund are distributed to all public K-12 schools in the state and are used by school libraries to purchase materials such as books, newspapers, and computers.

Allegations and Actions Against Agents
Vivian Bedolla, 6121 Green Bay Rd., Ste. 230, Kenosha, WI 53142, agreed to a permanent surrender of her Wisconsin insurance intermediary license. This action was taken based on allegations of failing to disclose an alias on a license application, making and causing to be made fraudulent communications regarding her license status, and obtaining or attempting to obtain a license through misrepresentations or fraud.
Mallory Cahill, 4907 Mountain Top Ct., Columbia, MO 65202, had her application for an insurance intermediary license denied. This action was taken based on allegations of having criminal convictions substantially related to the circumstances of holding an insurance license, having an administrative action taken by another state, and failing to disclose that administrative action.
Jason M. Coker, 7625 18th Ave., Kenosha, WI 53142, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Valencia S.Y. Coston, 36033 W. Madrid Ave., Maricopa, AZ 85138-5462, had her insurance intermediary license revoked. This action was taken based on owing delinquent Wisconsin taxes.
Deja Fields, 20513 Mcgilvary Dr., Crest Hill, IL 60403, had her insurance intermediary license revoked. This action was taken based on allegations of violating Wisconsin insurance law.
Daniel Forney, 4207 S. Dale Mabery Hwy., Unit 11108, Tampa, FL 33611, was ordered to pay a forfeiture of $7,000.00. This action was taken based on evidence of the respondent failing to properly consider suitability in selling health insurance, selling unauthorized health insurance in Wisconsin, providing false and misleading information to Wisconsin consumers, and for failing to respond promptly to inquiries from OCI.
Sterling Foster, P.O. Box 660042, Dallas, TX 75266, had her application for an insurance license denied. This action was taken based on allegations of failing to disclose a criminal conviction and pending criminal case, and having criminal convictions substantially related to the circumstances of holding an insurance license.
Gregory A. Hovey, 2114 W. Grant, #96, Tucson, AZ 85745, had his insurance intermediary license revoked. This action was taken based on allegations of falsifying consumer address information on insurance applications and failing to participate as ordered in an administrative hearing.
Charles Hughes, 3504 S.W. 4th St., Apt. B, Deerfield Beach, FL 33442, had his application for an insurance intermediary license denied. This action was taken based on allegations of having administrative actions taken by other states, having an insurance license denied, suspended, or revoked in other states, and violating insurance laws or regulations in another state.
Monica Klempel, 1415 Keystone Ave., River Forest, IL 60305, agreed to a two-year suspension of her Wisconsin non-resident insurance intermediary license and to pay a forfeiture of $20,000.00. These actions were taken based on allegations of misrepresenting insurance policy benefits.
Eugene Knapp, 7975 County Rd. K., Gratiot, WI 53541-9741, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Lafanear Mouton, 2144 N. 40th St., Milwaukee, WI 53208, had his insurance intermediary license revoked. This action was taken based on allegations of owing delinquent Wisconsin taxes.
Candice L. Palmer, 5037 Spruce Ct., Greendale, WI 53129-2960, had her insurance intermediary license revoked. This action was taken based on owing delinquent Wisconsin taxes.
Matthew Ricci, 260 1st Ave. S., Ste. 200, St. Petersburg, FL 33701-4364, had his Wisconsin intermediary license revoked. This action was taken based on allegations of failure to pay a forfeiture when due and because proffered payment check bounced, and not responding.
Jessica Rodriguez, 6424 NW 5th Way, Ft. Lauderdale, FL 33309-6112, was ordered to pay a forfeiture of $4,000.00. This action was taken based on allegations of failing to respond to the multiple attempts Wisconsin OCI made to communicate with her; for enrolling a Wisconsin resident in an insurance plan when she did not hold a license to do so in Wisconsin; for knowingly providing that Wisconsin resident with false information regarding that plan’s benefits; and for knowingly and fraudulently providing another agent’s NPN on the plan’s application.
Michael Sanguily, 9132 SW 143rd Ave., Miami, FL 33186-7822, was ordered to pay a forfeiture of $1,500.00. This action was taken based on allegations of making misrepresentations in the sale of an insurance product.
Allegations and Actions Against Companies
CareSource (Common Ground Healthcare Cooperative), 300 N. Executive Dr., Ste. 300, Brookfield, Wisconsin 53005, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of accepting insurance business transacted by an agent not appointed by the company.
United Services Automobile Association, 9800 Fredericksburg Rd., San Antonio, TX, 78288, was ordered to pay a forfeiture of $1,000.00 and pay the value of a roofing claim. These actions were taken based on allegations of not having reasonable standards for investigation of claims under their insurance policies.
Zurich American Insurance Company, 1299 Zurich Way, Shamburg, IL 60196-1056, was ordered to pay a forfeiture of $1,000.00. This action was taken based on allegations of accepting insurance business transacted by an agent not appointed by the company.
Allegations and Actions Against Other Entities
Auto Dent Pros, 7960 Rodeo Trl., Ste. 420, Mansfield, TX 76063, was ordered to pay a forfeiture of $1,000.00 and cease and desist from issuing advertising flyers with insurance advice. These actions were taken based on allegations of giving insurance advice without an intermediary license.
Alan P. Cooper, 2566 N. 60th St., Milwaukee, WI 53210, was ordered to pay a forfeiture of $100.00. This action was taken based on evidence of the respondent committing a fraudulent insurance act by knowingly presenting a false claim for payment of a loss.
